As of September 1, 2020 Summit Medical Group has a new and improved patient portal. This portal will allow patients to request and view appointments, send messages or refill requests and view test results and billing online without having to schedule an appoint ment or wait on the phone. After September 1, Summit will no longer update patient records on the ‘FollowMyHealth’ portal, but patients can still access their pre-September 2020 personal medical history by visiting the Follow My Health website. Test results are released to a patient’s Summit Health MyChart account after their provider has reviewed them or after 3 days regardless if the provider has reviewed the results. Tests that are sensitive nature may not be released to Summit Health MyChart.
Your provider is able to determine which types of test results are able to be accessed through MyChart. Further, tests of a very sensitive nature are not released to MyChart, however, clinical staff will contact you with results.

Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during your next office visit.

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.
Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.
Follow My Health Patient Portal will allow you to access your health information, schedule appointments, send notes to your provider and view your medical visits after your first visit with your provider. There are a few things you will need to know about the new Follow My Health Portal.
If you have your email on file with Summit Healthcare you will receive or have received an email inviting you to join the Follow My Health Portal. Please sign up but DO NOT enter any of your medical history into the portal. Once you check-in for your first appointment, you will be directed to complete your medical history. This is where you will have the opportunity to complete your medical history. After your first visit with your healthcare provider your information will automatically load into your Follow My Health Account.
Your provider will automatically populate once you have made an appointment with your healthcare provider. You do not need to search for your healthcare provider.
Summit Healthcare also has the capability and security to allow you to manage your health information through a third-party applications, such as Medlio, MyLinks or another of your choice that meets the technical specifications. When downloading the app to your smartphone or tablet your identity will be authenticated using your secure FollowMyHealth account. You can find the list of apps to choose from here.
No, when you register for the new Follow My Health Portal please be aware that none of your medical history from the previous health portal will be transferred over to the Follow My Health Portal. Rest assured your previous medical visits and your medical records from the former portal are still available to you and your healthcare provider. Simply open the app, scroll to Health Records under Health Data and search for "AMITA." Select your site and log in with your existing portal credentials.

Yes. Patients can create a family login account that will allow them to access selected family members' health information within the same login account. Family members will have the same access to patient information and actions. In order to setup an account of this nature, log into each patient's portal account.
