Does Northwest Texas Healthcare System keep medical records?

Northwest Texas Healthcare System retains your medical records in the Health Information Management Department. These records are kept in strict confidence and are not released without the written consent of the patient, except as required by law.

What is health records online?

Health Records Online enables you to manage your health anytime using your computer, smartphone or tablet. Connect with your doctors no matter where you are, at home, around town, or out of state.

Can you share your health records with other providers?

No. Only you may elect to share summaries of your care with other healthcare providers. We do not share your personal information and strictly follow our Privacy Policy. You may read that policy online or when you enroll in Health Records Online.

Do you charge for copying medical records?

Fees: No fee will be charged for records copied at the request of a healthcare provider or sent to a healthcare provider at the request of a patient for the purpose of continuing care. There is a fee charged for the search and duplication of medical records for personal reasons, attorney requests or insurance requests.

Paper Copy of Medical Records

Please complete the following steps to obtain a paper copy of your medical records:

Per Florida statute, there may be a charge for providing the copies at $0.25 per printed page and $0.02 per page for electronic copies. Records will be mailed within 5-7 business days.

Urgent Requests, Records for your Physician

For immediate continuity of care, your healthcare provider can request records.

Insurance, Attorney, Disability and Third Party Requests

Requests should be sent from your insurance company, attorney, or Disability Determination Service and mailed to the address below. We cannot accept faxes and email requests.
