How to get lab results from MyHealthHome?

Get your lab results by reading the directions on the portal home page. Please contact your pharmacy for prescription refills. Allow 24 to 48 hours for prescription refills to be completed. MyHealthHome is NOT to be used for urgent or emergency communications.

Is North Point Primary Care a member of the network?

North Point Primary Care is a member of a network of physician practices that utilizes MyHealthHome, a secure, web based system that allows you to contact our doctor electronically. MyHealthHome is available 24 hours a day for you to review your labs and conduct non-urgent communications with our office.

PrimeCare Fulton

Joselito Ouano, MD, is a board-certified family medicine physician who earned his medical degree from the Cebu Institute of Medicine in the Philippines. His residency was completed at The Reading Hospital in Pennsylvania. Dr. Ouano is also a graduate of Pennsylvania State University, where he obtained his bachelor’s degree in biology.

Lakeview Primary Care

Licensed in 2010 as a registered nurse, Nathaniel Farden, NP, earned his BSN in 2012 from Western University. Farden joined Oswego Health as an RN in the emergency department in 2015, then later transitioned as a utilization RN in case management in 2016 while continuing his schooling.

Oswego Family Physicians

Douglas Guenter, MD, earned his bachelor’s degree from Bethel College in St. Paul, Minnesota, and his medical degree from the Medical College of Wisconsin. Dr. Guenter served seven years in the U.S. Air Force and obtained the rank of major.
