Your First Visit

Your first visit with an ENA provider will include a physical examination and evaluation of your medical history including your current problem, your family’s medical history, and current medications. We will also obtain the necessary urine and blood tests at this visit.

Lab Work

If you’re having lab work done, we have collection stations conveniently located at each of our offices. Days and hours of collection can vary by office, so be sure to check your office’s schedule here.

Patients with Chronic Kidney Disease (CKD) are among the most vulnerable people in America. And, people with End State Kidney Disease (ESKD) can become some of the most medically complex patients and their treatment can be very costly.

Telemedicine allows you to visit with your provider without coming to the office.

We accept most insurance programs. If you have a question, call your ENA office.

We make it easy to access your health records, track your progress, message your provider and more online through our secure patient portal, MyChart.

How much is a no show fee for a missed appointment?

If you are unable to keep your appointment, please give our office a minimum of 24 hours’ notice. If 24 hours’ notice is not given, a $25.00 no show fee will be charged for missed appointments.

What to bring to a first visit?

What to Bring to Your First Visit 1 New patient materials including Patient Registration Form, Notice of Privacy Practices, Consent Signature, and Health History form. We will mail these to you in advance of your appointment, or they can be printed directly off our web site from the FORMs tab. Please complete these in advance or arrive 15 minutes early to do so. 2 Your current medication bottles (including all prescriptions and over-the-counter medications). 3 Insurance cards. 4 Photo-ID. 5 Co-pay, if applicable. 6 Patients might be required to provide a urine sample for kidney related testing.
