What is campus health services?

Campus Health Services is a resource for services and information to keep your student’s mind and body healthy while at NAU. Campus Health Services is committed to the concept of wellness. We hope you view us as a center for wellness as well as a place for your student to come when they are ill or in need of care.

What is Northern Arizona University doing to protect my medical information?

The preferred method is electronically. Go to the Campus Health Portal and enter your NAU credentials. Then click the “Immunization Requirement” link and follow the instructions. You will need to upload scans of your MMR documentation.

Immunization requirement – MMR (Measles, Mumps, and Rubella)

To safeguard the health of the NAU population, ALL new and transfer students are required to provide proof of immunity to measles, mumps and rubella (MMR) – for all students born after December 31, 1956.

MMR FAQs

Northern Arizona University requires that all students attending the Flagstaff mountain campus, including Coconino Community College students living in a NAU residence hall, provide medical documentation of their immunization or immunity to measles, mumps, and rubella (“MMR”).

Submission requirements

Flagstaff Campus students must only submit a completed Immunization Policy Form and proof of one of the following to Medical Services:

Please fill out the Telehealth Informed Consents for Medical or Counseling if you plan to utilize services.

Billing and Insurance

Telemedicine visits are billable services but all insurance plans may not cover telemedicine, even in these changing times.

Scheduling an appointment

Medical appointments can be made my calling or by logging into your Campus Health Patient Portal and scheduling online. Counseling appointments must be scheduled by calling the number below.
