The MEDENT Patient Portal provides you with secure, online access to portions of your medical records, as well as an easy and convenient way to communicate with the practice through a secure internet connection. Think of it as an online account for your doctor's office, similar to online banking or credit card accounts you may currently have.

Any active patient may be eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access selected family members’ health information.
After regular office hours, on weekends, or on holidays, your call to our office will be answered by a professional answering service and all emergency related calls will be relayed to your physician. Your doctor, or the doctor on call, will return your call as soon as possible. All non-emergent calls, such as prescription refills, insurance referrals or scheduling/canceling of appointments should be done during regular office hours. In the event of a serious or life- threatening medical emergency, please go directly to the emergency department at the nearest hospital covered under your insurance plan.
Your medical records are held in strict confidence. If you would like your insurance company, your attorney, or other third party to receive information from your records, we must receive a written request with your signed and dated authorization.
The Family Practice Center, PC, Patient Portal provides secure access to portions of your medical record and allows easy and convenient communication with our practice. So that we can provide this service without compromising your private information, no one will have access to your personal portal account except you.
For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.
