It is very important that you let the scheduler know about all the concerns you would like addressed during your appointment. This helps us make sure an appropriate amount of time is scheduled.
Any prescription medication you are currently taking. Bring in your medications or write them down and bring a complete up-to-date list.
We will provide you with all necessary paperwork to sign during your first appointment. You may preview them via our Patient Forms page.
We use an automated appointment reminder system that will call you one day before your appointment. If you schedule your appointment online, we will send you an email appointment confirmation and reminder a couple of days before the appointment.
In most cases you can expect to see your own doctor. However, there may be occasions when your doctor is at the hospital delivering babies or handling an emergency. If this occurs, we will call to reschedule your appointment or ask if you would be willing to see another provider at your scheduled appointment time.

Not one for talking on the phone? We get it, and with mySFM you can schedule your appointment with your provider right from your computer, tablet, or phone. And if for some reason you need to cancel an appointment you scheduled online, you can do that from mySFM too.
Have a quick question for your provider? With mySFM, you can send a message to your provider team and your provider or their medical assistant will be able to respond to you through the secure portal. The messaging center is quick, direct, and you can even send pictures.
While mySFM can’t refill medications for you ( your local pharmacy app can), if you’re needing a refill on your prescription, you can do that through mySFM. You may need to come in for a follow up appointment before a refill, but lucky for you, you can schedule that on mySFM, too.
Sometimes phone calls can be too much, so instead of calling you with lab results, mySFM allows you to view and read when your labs come back. Your provider might still give you a ring, depending on your lab, but if your preference is an online message, that’s what we’ll do.
Have a bill you need to take care of? No worries, you can pay it from the comfort of your home here. After equipping yourself with a credit/debit card and your bill, our portal will walk you through bill payment and send you a receipt.
Wondering about your medications, previous visits or lab reports? Wonder no longer, our portal gives you access to your medical records. Health records are available for viewing online from the day you register with mySFM to the present.
We get it, paperwork is tiresome. Our suggestion: skip the forms in the waiting room and type it out before you go. You can fill out a medical release for a minor child, review our financial policy, complete a patient profile and more all from your own device. Your Health Forms update online as often as you update them, either via paper or mySFM.
