In light of the increase in cases and hospitalizations due to COVID-19 in our community, the Fairchild Medical Center Foundation has made the difficult decision to cancel our 2021 Golf Tournament scheduled for September 12th at Lake Shastina Golf Resort. As a foundation we are focused on supporting the important work of our hospital and will continue to prioritize the needs of our team and patients. If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.
