What is patient portal?

The Patient Portal is a secure, confidential, and easy to use website that gives you 24 hour access to your personal health information. It uses the latest encryption technology to deliver secure communication between you and your providers. You can schedule non-urgent appointments, access your medical records, request a referral and more.

Can you use the patient portal for emergency?

The patient portal is designed for routine health management. It should not be used for emergency-related questions. If you have an emergency or other urgent matter, call 911.

Do you have to be present at the time of enrollment for a dependent?

To protect the privacy of all of our patients, if access is requested for any dependents/care managers to your portal account, it is required for you and the requested dependents/care managers to be present in the health center at the time of enrollment. Enrollment for patient’s ages 0-17 and conservators of adult patients must be completed in person at one of our clinic locations. If you are interested in signing up please let us know during your next visit.

Does San Ysidro send health information?

No health information is sent via email. When a message is sent from the doctor’s office, you receive an email stating you have a new message from San Ysidro Health and are directed to log in to San Ysidro Health Patient Portal to review.

For Magnolia Regional Health Center’s MyHealth Patient Portal users, your health information may also be accessed through an application such as those used on a smartphone. Depending on the chosen application, you may be able to access information such as your medications, allergies, or lab results. If you are interested in using an application, please contact our helpline at 662-293-1007. Our staff is available to help between the hours of 8 am and 5 pm, Monday through Friday.

How to enroll in MyFHN?

MyFHN is an online tool that helps you manage your healthcare easily and securely. To enroll, click on the Enroll button to the right ; there are two options available for enrollment.

What is a proxy authorization form?

A Proxy Authorization Form is required to access the health information of anyone over age 12 by someone other than themselves. Please note that all signatures must be witnessed by an FHN staff member. If you have any questions about this process, please contact the MyFHN Coordinator at 815-599-6907 or toll-free at 1-877-6000-FHN (1-877-600-0346) ext. 907. To revoke proxy permission, a Proxy Revocation form must be completed.
