If you do not have an appointment, contact us at 905-494-2120 ext. 58864. Follow the link provided in your email or visit MyChart to complete your registration.
If you are an Osler patient, you can securely access your clinical health information online through the MyChart™ patient portal. Once registered, you can use MyChart™ to review your test results and share your health information with family, caregivers, medical professionals and pharmacists.

All Osler patients can register for MyChart™. If a patient is younger than 14, their parent or legal guardian can contact us at 905-494-2120 ext. 56203 to access their child's records. When children turn 14, they take ownership of their own MyChart™ accounts.
