The NIH Clinical Center's FollowMyHealth® Patient Portal is an internet-accessible application that offers patients secure access to pieces of their NIH Clinical Center electronic medical record.
Patient portal apps typically allow patients to share their health records with providers, enabling them to gain a longitudinal view of their patient's health history. Like Epic's MyChart, some apps even allow patients to provide temporary access to their medical records via a code.
Step 1: Go to https://www.followmyhealth.com (link will open in a new tab or window). Step 2: Click the “I need to sign up” button (circled in red in the image below). Step 3: Fill in the appropriate information seen in the next image below. Then select “Confirm and Continue” at the bottom of the page.

This is due to a lack of internet access. According to the AMA, 25% of people don't use a patient portal because they don't have internet access. Over one in six people in poverty don't have internet access. Lower-income people in rural areas face even more limitations.
Among individuals who were offered a patient portal, about eight in 10 were encouraged by their health care provider to use it. Seventy-one percent of individuals encouraged by their health care provider accessed their portal at least once in the past year compared to 48 percent who were not encouraged.
Step 1: Sign in to myGov Go to the myGov sign in page. If you don't have myGov account, click 'Create an account' and follow the steps to register. If you already have a myGov account, use your existing username and password to sign in.
Check for updates to the app. It could start running slowly or some things within the app may not function properly. Be sure to install the latest updates to keep the app functioning properly. FollowMyHealth™ will notify you if you sign into the app and there is an update available.
TIP: If you are having trouble logging in to your account, use our username recovery tool. Step 2: Under the “Sign In Preferences” section, click on the row that says “Create FollowMyHealth Username and Password”. Step 3: A window opens that allows you to create a new username and password.
To delete your account, click “My Account” in the upper right-hand corner of your FollowMyHealth homepage. Then, click on “Preferences.” Then, click “Delete your UHR” under “Account Preferences.” The system will prompt you to confirm once more. If you so choose, click “Yes,” and your account will no longer be active.
FollowMyHealth does not store your username and password for these particular providers and your credentials will continue to be maintained by them as well. In compliance with federal patient privacy regulations, all transmitted data is encrypted during this process. The data storage is also encrypted for protection.
While in the Northwell Health app, tap the "Me" icon in the bottom navigation menu to enter your profile. Once in your profile, you will see a button to connect to FollowMyHealth. Or you can connect to FollowMyHealth here.
How to view your lab results from your mobile device: 1. Open your FollowMyHealth® app and log in to your account. 2. Under the Action Center tab, click the link that says results were added to your health record.
