Where does the HEALTHECHART patient portal come from?

All of the information in your HEALTHeChart Patient Health Portal comes directly from your Mercy Hospital Medical Record . This ensures that you have access to the most accurate, up-to-date information possible.

Email HEALTHeChart@cantonmercy.org and ask to be enrolled in the HEALTHeChart patient portal.

How old do you have to be to get a health chart?

A: Patients must be 18 years or older to sign up for a HEALTHeChart account. If you are under 18 years of age, your parent or legal guardian will need to establish a connection to you via proxy access. Please visit the medical records department with your parent or legal guardian. Once you turn 18, you will automatically disconnect from your parent or legal guardian’s account and you may sign up for your own account.

What is a HEALTHECHART?

A: HEALTHeChart is a secure online tool that connects you to your personal health record. After you enroll, you can connect to it through Mercy Hospital’s w ebsite: www.cantonmercy.org/healthechart.

Can you send confidential information through HEALTHECHART?

A: HEALTHeChart is not an email system and we do not recommend sending any confidential information through standard Internet email. When you activate your personal HEALTHeChart account, you will be asked to provide your email address so that you can receive email notification to log on to your secure HEALTHeChart account whenever new information has been made available. Any confidential patient information transmitted from your Mercy health care team will be located in secure messages that you will see only after you have logged into your secure HEALTHeChart account.

Is HEALTHECHART an email?

However, HEALTHeChart is not an email system and it is not intended for communication with your physician.

Can you access a healthheart?

A: Only you – or a person of your choosing – can access your HEALTHeChart . HEALTHeChart can be used by legal guardians or caregivers with durable power of attorney for health care of dependent adults to access portions of the medical record of their loved ones. A HEALTHeChart proxy form is available to allow parents or legal guardians access to a patient’s online medical record.

Where is the signature on a patient authorization form?

Fully complete the authorization request in detail, including the patient’s signature and date of signature, located at the bottom of the form. If the patient is unable to sign, please provide the appropriate legal documentation needed to prevent any delays in retrieving records.
