What is the Tri-State wellness program?

Make sure you have given us an e-mail or phone number where you may be reached with either question or to schedule your appointment. If you decided you would prefer to have paper copies of these forms, please call 859-655-4480 and a packet will be mailed to you. Please bring your insurance card to our office on the day of your appointment.
