Patients may request a copy of their medical records by calling (317) 885-2860. A Release of Information Authorization form must first be completed by the patient or legal guardian prior to requesting medical records.
The patient always has the option of calling 911 or going to the nearest hospital emergency room should they feel they have a medical condition that warrants immediate medical attention.
PLEASE NOTE: If a patient has granted Power of Attorney to someone, we must see the Power of Attorney before processing the request for medical records.

Beginning May 4th, 2021, our Practice will be implementing a new patient portal. If you are enrolled in our current Intelichart patient portal, you will have to re-register and create a new portal account. On that date, we will begin enrolling patients, at their office visits, for the new patient portal.
