What is Syracuse Community Health Center?

(SCHC) plays a critical role in assuring access to health care for all of the residents of Onondaga County, especially those who are uninsured or who experience other barriers to care. SCHC provides primary health and supportive care services to more than 34,000 unique patients regardless of their ability to pay for services.

What is a SCHC?

Syracuse Community Health Center (SCHC) is a patient-centered medical home which means our focus is on you and your health. In a patient-centered medical home, a team of health professionals works together to create an environment where you have access to quality health care that provides you with the best possible health outcomes. Your primary care provider leads the care team which may include nurses, medical assistants, receptionists, specialists and other health care professionals working with you as the center of the team.
