Once you have canceled, renewed, and/or added all your caregivers, click the MCPP Patient Registration tab, then Save, and finally Continue to progress the Renewal Application process. Note: Once you click on Save and click on Continue, you will not be able change your information (except for gender, email, and phone number).

MPCP is enrolling patients from all of our locations who have a current, active email address on file with the practice into our new MPCP MyPortfolio patient portal. If you have not yet signed up for the MyPortfolio patient portal, please click on the MyPortfolio button below and then click on Sign Up Now.
If you are not on the MCPP Patient Caregiver tab, select the MCPP Patient Caregiver tab. Select the caregiver from the caregiver list and select Cancel Caregiver. A canceled caregiver during the extension or renewal process will cancel your caregiver’s future card.
In August 2013, Illinois became the 20th state to authorize a program for the cultivation and dispensing of cannabis for medical purposes. The Medical Cannabis Registry Program is a statewide program that allows patients access to medical marijuana who meet one of the qualifying conditions.
Monthly Medical Cannabis Patient Program data can be found on the Statewide Medical Cannabis Pilot Program website.

If you want to continue with the MCPP program, submit an extension or renewal prior to the card expiring. Refer to the Qualifying Patient Extension instructions for the extension process. An email will be sent to you when it is time to renew your card. Sign into the system using your username and password.
You need to select a 1-year, 2-years’, or 3-years’ timeframe for the renewal application. Note: If your medical condition is lifelong, the timeframe will be automatically set by the system for a three-years term. You will have to answer the caregiver question.
