How do I obtain a copy of my MCMC health information?

If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk to help you regain access to your MyChart account. OHSU and Tuality patients call 503-494-5252. MCMC patients call 541-506-6499.

What services does OHSU provide to patients and their families?

MCMC Encourages Patients to Sign-Up For MyChart. MyChart provides MCMC patients with secure online access to some medical records, allows you to track your health information and communicate with your medical office. In the near future, MyChart will be used to send patients the links to their Telemedicine appointments.

What is OHSU connect and how does it work?

Mail to MCMC Health Information Management, 1700 East 19th Street, The Dalles OR 97058. Fax to MCMC Health Information Management at 541-296-7617. We will then assess whether what you request is available, and prepare a copy. You may either pick it up from our office at the above address, Monday through Friday 8-4:30 PM, or we will mail it to you.

How do you get to OHSU MyChart?

Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during your next office visit.
