On the Check Member Eligibility panel, select the provider from the drop-down list. Enter either the member's: 12-digit Member ID. SSN. Other Agency ID (DCF and DYS) First name, last name, DOB and gender. In the From Date of Service and To Date of Service fields, enter the date range for the search. Click Submit.
Use these services to enroll as a MassHealth provider, manage your profile information, and submit and retrieve transactions. Renewing your plan will usually happen one year from your sign up date. MassHealth will mail you a letter with your renewal date and instructions before your coverage expires. Make sure that you keep your mailing address up to date for BMC HealthNet Plan and MassHealth so that you do not miss this notice.

All of the pages in MAP contain phone numbers for you to call if, for example, you have any questions about registering for a Virtual Gateway account, accessing or moving around in MAP, or the information you see in MAP.
