Maimonides Medical Center protects the confidentiality of every patient’s medical record. As a patient, you have the right to access your medical records. You may access your available records on the FollowMyHealth® Patient Portal that allows you to conveniently manage your health information. With your Maimonides account, you are able to:

Visiting will be limited to the following hours: Sunday – Friday, 9am—9pm*. Saturday, 9am—10pm *These hours may be subject to change. One visitor will be permitted at bedside, with additional visitors allowed at the discretion and with approval of the patient’s medical team. ED visitation is available 24 hours/day.
Two family members and/or legal representatives may be permitted at the bedside of any patient who is actively dying, or whose death is anticipated within less than 24 hours. The patient or the family/caregiver may designate up to two support people.

With the FollowMyHealth® Patient Portal, you can access your personal medical records—from Maimonides and other medical providers. You may do the same for your children who are younger than 12 and for adult family members with impaired decision-making abilities.
Except for end-of-life situations in which the patient is the parent of a minor child, children under the age of 12 are prohibited from visitation. The Medical Center reserves the right to ask any visitor to leave if they interfere with the clinical care of the patient or other patients.
Visitors must remain in the patient’s room for the duration of the visit except for entering and exiting the hospital. Visitors who fail to adhere to this protocol will be asked to leave the Medical Center immediately (NO exceptions) and future visitation within the Medical Center may be restricted.
There is an existing court order restricting contact. The healthcare professional may limit the visitation based on clinically reasonable considerations. The facility may limit the number of individuals visiting a patient based upon space, patient condition or rights of other patients.
Patient information and data may only be released upon receipt of a written request with appropriate patient authorization, valid subpoena, and a court order or as otherwise required by law enforcement.
Your physician may submit a request for your records to the Medical Records Department.
Requests for treatments such as going to a follow-up appointment, consultation or another healthcare facility will be expedited.
Records can be sent electronically or paper copy via certified or registered mail (i.e., USPS/FEDEX/UPS).
They will be sent as requested in the request form unless you define that you would pick them up in person.
Please provide the supporting documentation to the Medical Records Department.

The Patient Relations Department works to ensure your safety and comfort. Our patient representatives educate you and your family about your rights and responsibilities, help you communicate with hospital staff, and handle care issues such as advance directives. Most patient representatives are bilingual or multilingual with fluency in Hebrew, Yiddish, Urdu, Spanish, Italian, German, Chinese, Arabic, Greek, or Russian. When we admit you to the hospital, we will ask you to sign a form required by the Health Insurance Portability and Accountability Act (HIPAA). HIPAA protects your medical records and other personal health information and sets limits and conditions on the uses and disclosures that may be made of such information without your permission. HIPAA also establishes your right to control your own health information, to examine and obtain a copy of your health records, and to request corrections.
