Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s healthcare team. When your child turns 12, you will not have access to any new information in your child’s portal.

Enrollment is fast and free, based upon information provided by your Med Center Health caregiver. You’ll need a name, date of birth, social security number, email address on file with your caregiver and your medical record number.
