How many parents can be allowed to stay overnight at a hospital?

Two parents, designated support persons or surrogates will be permitted access to the patient at all times, and these individuals may rotate. Extended overnight stay (9pm-9am) is permitted for one adult.

Can a companion be accompanied to the hospital?

Patients undergoing same-day procedures may be accompanied to the facility by a companion and that companion may remain with the patient through the initial intake. During the procedure, the companion may wait in the designated Surgical Visitors Waiting Area until the patient is discharged, unless allowed by the Medical Center due to the patient’s status or condition.

Can you access your medical records with FollowMyHealth?

With the FollowMyHealth® Patient Portal, you can access your personal medical records—from Maimonides and other medical providers. You may do the same for your children who are younger than 12 and for adult family members with impaired decision-making abilities.

What are advance directives?

The 3 types of advance directives are: 1 Do-not-resuscitate (DNR) order – A form that instructs medical staff not to try to revive you if your breathing or heartbeat stops 2 Health care proxy – The person you appoint to make treatment decisions for you, including your wishes regarding organ donation 3 Living will – A document that states your specific instructions and choices regarding medical treatment

What is an ECG?

An electrocardiogram (ECG) or x-rays. A review of your medical history, all the medications you are taking, and advance directives such as a healthcare proxy or living will. Tests of your blood, blood pressure, pulse, and other vital signs.

What is the HIPAA form?

When we admit you to the hospital, we will ask you to sign a form required by the Health Insurance Portability and Accountability Act (HIPAA). HIPAA protects your medical records and other personal health information and sets limits and conditions on the uses and disclosures that may be made of such information without your permission. HIPAA also establishes your right to control your own health information, to examine and obtain a copy of your health records, and to request corrections. These policies are available in multiple languages.

What is the patient relations department?

The Patient Relations Department works to ensure your safety and comfort. Our patient representatives educate you and your family about your rights and responsibilities, help you communicate with hospital staff, and handle care issues such as advance directives. Most patient representatives are bilingual or multilingual with fluency in Hebrew, Yiddish, Urdu, Spanish, Italian, German, Chinese, Arabic, Greek, or Russian. Our patient representatives are available in the emergency room 24/7 and on inpatient units from 8 a.m. – 12 a.m., 7 days a week. You can reach the Patient Relations Department at (718) 283-7212.

What languages are required to provide a copy of the Patient Bill of Rights?

We can provide versions of this notice in Spanish, Russian, Creole, Korean, Chinese, or Italian.

What to do if you don't understand the hospital?

If for any reason you do not understand, or you need help, the hospital MUST provide assistance, including an interpreter. Receive treatment without discrimination as to race, color, religion, sex, national origin, disability, sexual orientation, source of payment, or age.

What is an abstract in medical records?

An abstract is a summary of the medical documentation required for treatment purposes (continuity of care) which includes, but isn’t limited to, the discharge note/instructions, procedure/operative reports, consultations, history, and physicals, labs, imaging studies, etc. Information in my medical record is wrong.

How old do you have to be to get a medical authorization?

An adult (18 years old or older): The patient or their lawfully appropriate representative can gain access to medical documents with documentation. A minor (under 18 years of age ): Complete and submit the Maimonides Medical Center Authorization form.

If you need assistance with the form, please contact the Medical Records department at (718) 283-7454. The completed form should be submitted to the Medical Records department. They can be sent via email, fax, mail or in person.
