Beginning June 15, 2019, Lake Regional patients access their health information using two portals. The below information is for the clinic patient portal. This new portal, called HEALTHConnect, provides information about clinic visits, as well as results of labs and diagnostic imaging related to clinic visits.

Lake Regional Health System is a member of the Tiger Institute Health Alliance Health Information Exchange. A health information exchange (HIE) assists with the electronic transfer and sharing of health related information between physicians, hospitals and other health care providers at the time it is needed to help them make clinical decisions.
NOTE: Records requests must be signed by the patient or the patient's legal representative and submitted electronically, by mail or by fax. Records will not be released without signed authorization from the patient or the patient's legal representative. A valid photo ID is required to pick up records.
