New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.
Payment for deductibles, co-payment, and/or co-insurance is expected at the time of service.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
On your statement under the Description column, you will see an asterisk (*) beside the line item detail which will indicate that insurance has been filed and charges are pending payment from your insurance carrier.

Yes, you can give family members such as parents or healthcare proxies, access to the Patient Portal. This needs to be done within your healthcare facility and requires consent from you and your family member (s).
Yes. Patient Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your portal. Also, a timeout feature protects your information if you leave the Patient Portal page open.
