The easy-to-use online tool helps you manage your health by connecting you with providers and giving you access to lab results, appointment information, video visits with providers, current medications, and more from your computer, tablet, or mobile device.

Use the MyChart patient portal to schedule an appointment, request an appointment or cancel an upcoming visit with your primary care physician. Prior to your scheduled visit with your provider, you can use eCheck-In to update insurance information, medications, allergies, fill out questionnaires and sign documents. Enrollment is easy and free

To enroll, all you need is a secure Personal Information Number, or PIN. We’ll send you a secure PIN by mail after your visit — or we can email it to you as a part of your registration process, if you prefer.

If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility or an activation code, you can still sign up for MyChart. If you've been a Baptist Health patient, your MyChart account will connect to your medical record.
