Lewandowski enjoys all areas of family medicine. In his personal time he enjoys skiing, golfing and his military flight time. Something everyone should know about him: He is currently a flight surgeon with the USAF and for 8 years he was a security specialist for the USAF.

Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call during regular business hours.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

Payment for deductibles, co-payment, and/or co-insurance is expected at the time of service.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

On your statement under the Description column, you will see an asterisk (*) beside the line item detail which will indicate that insurance has been filed and charges are pending payment from your insurance carrier.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

We are happy to help with refill requests during regular office hours, so please call us at to let us know what you need.

Pediatrics

Pediatricians are specialists in the care of infants, children, and adolescents. We currently accept new patients under the age of 18. Our training consists of three years of residency, limited to pediatrics, after completion of medical school.

Internal Medicine

Internal Medicine doctors specialize in adolescent and adult medicine. Most would consider us primary care physicians that are specifically trained in taking care of adult routine care problems in addition to more complex medical issues and coordinating specialists as the need arises. Our practice will see individuals age 14 and older.
