Our Patient Portal gives you secure, online access to your health information. Patient Portal is a secure, convenient, and easy-to-use website that gives you round-the-clock access to your health information. With Patient Portal, you can view labs, medications, immunization records, statements, and much more.

Do I have to be a Denver Health patient to access MyChart?

Appointments & Cancellations

Our office is open Monday thru Friday, 8 AM to 5 PM. You may schedule an appointment thru My Health Record or by calling (602) 867-2690. If you need to cancel or reschedule an appointment you must do so at least 24 hours prior to your scheduled visit.

Payments

As a service to our patients, we submit all primary insurance claims. We submit secondary insurance claims for patients. Copays are expected to be paid at the time of service. We encourage our patients to contact your insurance carrier to determine what is covered by your plan.

Insurance Forms

Our fees are determined by the individual care given and are competitive with area specialists. Not all office visits are covered by your insurance policy so it’s important to be familiar with your coverage. ​
