A health information exchange (HIE) assists with the electronic transfer and sharing of health-related information between physicians, hospitals and other health care providers at the time it is needed to help them make clinical decisions.
An HIE is particularly helpful in emergencies, when patients often cannot provide such health information as important diagnoses, current prescriptions and allergies. Because patients are in charge of their health records, they do have the choice to “opt out” of the exchange.
Your health information comes directly from your electronic medical record at the hospital or doctor’s office. If you have questions about results or medical information, please use the messaging function to alert your provider or you may call your provider’s office directly.
Proxy access allows a parent, guardian or other designated individual (like a spouse, family member or friend) to use a patient portal to connect to information regarding a family member or friend. If you are a parent who wants to use one or both of our patient portals to view your child’s health information, you will need proxy access. Learn more on our Viewing a Family Member's Information page.
If you have questions or concerns about the quality of care you or a family member received at our clinic or pharmacy, please contact the location manager or Lake Regional Medical Group administration at 573.302.2765.
To be eligible for Lake Regional's Chronic Care Management program, patients must have two or more chronic conditions, such as arthritis, COPD, heart disease, cancer, diabetes, obesity, depression or some other ongoing medical problem. Participants also must have a Lake Regional primary care provider and be enrolled in Medicare.
Primary care clinics provide sick visits for such needs as a sore throat or fever, as well as management of chronic conditions, such as diabetes and high blood pressure. Primary care clinics also provide well-child checks, sports physicals and preventive medicine for adults.
Medicare covers a full range of preventive services to help keep you healthy and help find problems early, when treatment is most effective. These services include cancer screenings, cardiovascular screenings, vaccinations, diabetes prevention and more. Ask your primary care doctor which services are right for you.
You may request an appointment with your primary care provider by using our online form. While in-person visits at all of our clinics are available and safe, you also have the option to see your provider from the comfort of your home. Choose “virtual appointment” under preferred location for this option. You will receive a reply shortly.
