How much notice do you need to cancel an appointment?
Appointments fill quickly, and cancelling with less than 24-hours notice does not allow enough time to schedule another patient in need of treatment. Therefore, a cancellation or no-show fee will apply if our office is not notified that you will be unable to make your appointment.
Do you need to complete patient information forms?
On or before your first visit, you will be required to complete patient information forms. This information is necessary to assist in filing your insurance or handling your particular situation. A consent to release information and authorization for your insurance company to pay our facility for services rendered to you are also included. All information you provide us is strickly confidential.
