Call the office to schedule an appointment

To become a new patient you must call the office at (706) 882-9341 to schedule your first appointment. You will be asked to provide some basic personal and insurance information, along with your email address.

Register with your PIN

After scheduling your first appointment, we will send you a one-time PIN enabling you to register to use our portal.
