On June 5, 2021, UK HealthCare will launch UK HealthCare MyChart, our new patient portal. (Learn how this transition will impact your UK HealthCare FollowMyHealth account .) UK HealthCare MyChart is a secure and easy-to-use online patient portal that allows you to manage your care and access your health information any time.
You can use our patient portal login, which is secure, to check your records, get in touch with your provider about health concerns and set up appointments. With our patient portal, you do not have to wait for normal clinic hours to have questions answered or access the information you need.
If you are having trouble logging into your account or reaching our patient portal, please contact your nearest KentuckyCare clinic. Our courteous and friendly staff will provide you with the assistance you need, so you can access your patient information.
Note: KRS 214.185 allows minors, in certain situations, to be treated without parental/guardian consent. Under HIPAA 45 CFR 164.502, this sometimes prevents UK HealthCare from releasing information to the parent/guardian.
UK HealthCare MyChart is a secure and easy-to-use online patient portal that allows you to manage your care and access your health information any time. MyChart provides you with online access to your health record. It can help you participate in your health care and communicate with your providers.
Proxy accounts for minor children and dependent adults. If you are the personal representative (for example, parent, legal guardian, attorney-in-fact) of a UK HealthCare patient, you may request access to the patient’s health information. This is known as proxy access .
MyChart is a free service offered to our patients.
You can also contact the HIM office through MyChart. (Select Messages from the top menu, hit the "Ask a Question” button and then select “HIM Document Question” in the dropdown menu.)

The patient portal is a secure, online website that gives patients convenient, 24-hour access to personal health information including a summary of your medical visit and test results. Patients must register for the portal to gain access and registration instructions are provided below.

After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
The “eCheck-In” activity will ask you to review and verify your personal information, just as you would if you were registering with the front desk. You will then be prompted to complete any medical questionnaires related to your visit. If at any time you cannot complete part of the process, you can cancel or tap “Finish Later” to leave the activity and save your answers. If you come back later to continue, you’ll be returned to where you left off.
For example, Baptist Health supports the Apple Health Records app on iPhone , which makes it easy for patients to see their available medical data from multiple providers whenever they choose.
