Requesting a Copy of Your Records

A copy of your medical records may be obtained by downloading and printing the form below and bringing it to your doctor's office with a copy of your ID, bringing it to Kent Hospital's Health Information Management (HIM), medical records office, which is on the first floor of the main hospital or by mail.
