Our office is pleased to provide you with online access to your health information through our Patient Portal. From here you can: After you click on the button below, you will be taken to the "Patient Portal". You can then enter your "User Name" and "Password", and press the "Login" button to continue.

How do I access patient medical information online?

Please Note: Online access to patient medical information for visits that occur on or after September 28, 2019 will be accessible through our new patient portal MyJeffersonHealth. Historical medical information will still be accessible from either the hospital portal or physician portals, respectively.
