The Metro Health Patient Portal is a secure online option that gives patients convenient, 24-hour access to personal health information from anywhere with an internet connection. The purpose of this portal is to support new and existing patients with records and information concerning Immunizations, STDs, and Tuberculosis.

What should I do if I have technical issues with patient portal?

You will need to contact your medical provider’s office for any technical issues. But before you do, please check that you’re attempting to access your patient portal from a desktop computer using a Chrome, Microsoft Edge, Mozilla Firefox, or Safari browser. Medical records are confidential and are protected by state and federal laws. Although the medical record is the property of San Antonio Regional Hospital, the patient has the right to access information contained in their medical record.

Birth Certificates

You may obtain a birth certificate by submitting an application at the San Bernardino County Recorder Hall of Records, 222 W. Hospitality Lane, San Bernardino, CA 92415-0022. The Hall of Records is open Monday – Friday, 8:00 am to 4:30 pm; closed weekends and holidays.

Radiology Imaging

If you would like a copy of your radiology studies, you can request a copy following the same process as above. Copies of radiology images are provided by the Health Information Management Department at the main hospital. Please complete the authorization form and send to the Health Information Management Department.

What are the outpatient services at the hospital?

A few outpatient services are exclusively provided at the main hospital; these include nuclear medicine, respiratory therapy, neurodiagnostic, and cardiology; all by appointment only.

Is cardiac rehabilitation open?

Cardiac rehabilitation is open weekdays by appointment only. Closed Saturday, Sunday, and holidays.

What to do if you have issues logging into your patient portal?

If you’re having issues logging into your patient portal, please contact your medical provider’s office for support.

Can you use a portal locator on a patient portal?

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.

Where is San Juan Regional Medical Center billing office?

This applies to our main hospital billing office as well as our billing offices at 300 West Arrington Street, Suite 101, and will continue until further notice.
