Medical Records. The Health Information Management Department is open Monday through Friday, 7:30 a.m. to 4:30. p.m, excluding holidays. All requests must be in writing. Authorization forms are available in the Health Information Department, located on the first floor of the hospital or they may be printed from this page.
Jackson Hospital is required by law to maintain the privacy and security of your medical record. We will not use or share your information other than as described in our Notice of Privacy Practice (NPP) unless you give us permission, and you may change your mind at any time.

Enroll in the Patient Portal by either completing the online self-enrollment process or by filling out an enrollment form and returning it to the JCMH registration area, the JCMH Health Information Management department or a JCMH Medical Clinic receptionist.
