What is a hospitalist service?

Hospitalist Service physicians collaborate with other members of the hospital team assisting in coordinating patients' care--both while patients are in the hospital, as well as preparing them for any outpatient care that's needed. Patients are admitted to the hospital, navigated through the health care system, and discharged back to their primary care doctors' care. If they do not have a primary care physician, they will receive a recommendation for follow-up and direction to a primary care physician. Our Home Visit Service will provide primary care ranging from the "routine physical" to care of multiple complex and serious conditions. This will be accomplished via examination, X-ray, EKG, blood work and urinalysis, as well as ultrasound studies, all done in the comfort of your own home. The Home Visit Service also works closely with area nursing agencies, therapists, social workers, podiatrists and dentists to provide comprehensive care that includes managing medications, wound care, hospice care and hospital follow-up.
