When you make an appointment, we can address the key concerns you may have. We will be focused on addressing those issues during an expanded 30 minute office visit.
So that we may provide your care as efficiently as possible, we ask that you bring the following items to your first appointment in our office:
If you must cancel your appointment, we’ll be happy to reschedule it to a time more convenient for you. We ask you to give us at least 24 hours advance notice in order to provide an opportunity for another patient to be seen.
If you do not have your insurance card and/or co-payments the day of the appointment, we reserve the right to reschedule your appointment to a time when you can provide documents or payments.

If you have refills remaining on your original prescription, please call your local pharmacy or mail-in pharmacy to receive your refills. IPC DOES NOT need to be called.
