What is “innovative” healthcare evolved?

Our urgent care, primary care and behavioral health teams work together to provide coordinated care for our patients. That’s Innovative. We call it Healthcare Evolved. provider? medical need? some things out? We are now seeing patients in person or via telemedicine.

What is innovative care?

Innovative Care is a medical group that uniquely brings primary care, urgent care and behavioral health together to serve our patients better. Here, you can walk-in for a sudden illness or injury seven days a week.

How do I get a preauthorization from ICM?

We utilize advanced and innovative technology that allows us to easily connect and partner with TPA’s throughout the United States. To submit a preauthorization request over the phone, please call the direct line at: 800-862-3338. What can I expect during my first visit?

When you make an appointment, we can address the key concerns you may have. We will be focused on addressing those issues during an expanded 30 minute office visit.

What do I need to bring to my first appointment?

So that we may provide your care as efficiently as possible, we ask that you bring the following items to your first appointment in our office:

What is your appointment cancellation policy?

If you must cancel your appointment, we’ll be happy to reschedule it to a time more convenient for you. We ask you to give us at least 24 hours advance notice in order to provide an opportunity for another patient to be seen.

What are your billing requirements?

If you do not have your insurance card and/or co-payments the day of the appointment, we reserve the right to reschedule your appointment to a time when you can provide documents or payments.

What should I do if I have a question about my bill?

For questions about your bill for a visit with your provider, please contact our office at 480.776.0626.

What do I do if my prescription runs out before my next doctor's office visit?

If you have refills remaining on your original prescription, please call your local pharmacy or mail-in pharmacy to receive your refills. IPC DOES NOT need to be called.
