How to access medical records?

Access to another patient’s medical information may be granted under the following conditions: 1 Parents or legal guardians who have children under the age of 18. (Access will automatically expire when the patient turns 18 years old.) 2 Parents or legal guardians who care for individuals with mental or physical impairments. 3 An individual gives his or her permission for another person to have access to their medical records information. 4 A Durable Power of Attorney for Healthcare has been activated and a copy is put on file at the hospital. (Please notify us immediately of any change in authority.)

What is discharge instructions?

Hospital discharge instructions. Create your own personal health record to track additional health-related activity or appointments. Access to another patient’s medical information may be granted under the following conditions: Parents or legal guardians who have children under the age of 18.

What does "no show" mean?

A “no-show” is the term we use when a patient misses an appointment without cancelling it within one (1) business day in advance. Unfortunately, “no-shows” inconvenience those patients who need access to medical care in a timely manner.

Can you refill narcotics by phone?

Narcotics will never be refilled by phone. Additionally, these must be picked up in person at the office and a signature is required. Please do not call outside of regular office hours for narcotic refills because we will be unable to fill them for you.

A fellow of the American College of Physicians and a member of the American Geriatric Society, Dr. Carlson has been practicing medicine for more than 30 years. After receiving his M.D.
