How do I prepare for an appointment with an imperial health physician?

You can also request a refill through our secure Patient Portal. Appointments & Referrals

Appointments can be scheduled for either of our locations by calling 337-721-7236 during regular business hours. You can also schedule an appointment online by filling out this form. Every effort will be made to schedule your appointment within a convenient time frame.

Medical Records

Your records are a permanent part of the medical records at the Center for Orthopaedics. We are mandated by state law to keep the original records, including x-rays.

Payment Policy

After you have been seen by the doctor, you will proceed to the check-out desk. We will file your claims to your insurance company. You will be responsible for your unmet deductible amount as well as any applicable co-pays and co-insurance amounts at the time of check-out.

Medicaid & Managed Care Insurance Policies

It is your responsibility to contact your insurance company to determine if authorization or a referral is needed. Referrals must be obtained prior to services being rendered. Please ensure that we have a referral on file when you check in.

Medicare

Medicare patients are responsible for any unmet deductible and co-insurance amounts.

Student Insurance

Student insurance is automatically considered secondary if you have other insurance coverage except for Medicaid. The patient has to provide us with a completed Claim Form prior to services being rendered.

Secondary Insurance

We file secondary insurance as a courtesy only, but it is the patient’s responsibility to notify our office if a secondary insurance claim is to be filed.

A conflict of interest, which might impair objectivity in any review or decision process, the physician shall not participate in any deliberation involving such issues and shall not cast a vote or any related issues. Decision-making is based only on appropriateness of care and service and existence of coverage.
