Simply call our office or send us an e-mail. We will be happy to schedule an appointment for you. When you call to schedule your appointment, our front office staff will request some basic information from you. We also offer same-day appointments.
Please arrive 15 minutes early to complete registration forms. To assist us, please bring your insurance card and a photo ID, as well as a list of any medications, vitamins and/or herbal supplements you are currently taking. If you are bringing your child, please bring his/her immunization records as well.
After you complete the necessary registration forms, we will take some time to get you acquainted with our office. We will discuss our financial policy with you and help set up a financial plan that is best for you. We are always available to answer any questions you may have concerning your account and/or treatment.
If your pharmacy asks if they may contact us for a refill, please allow them to do so and you need not call us.
