New Applicants: Select +New MCPP Patient Registration to begin a MCPP application. If you are not sure which program to apply for, talk to your health care provider before starting the application.

Starting MCPP Caregiver Application

To add a caregiver, select the MCPP Patient Caregiver tab and select + New to begin the caregiver application. Before adding a caregiver, make sure you have answered Yes to the caregiver question in the Patient Application section. Note you can have a maximum of three caregivers.

Renewal Instructions

If you want to continue with the MCPP program, submit an extension or renewal prior to the card expiring. Refer to the Qualifying Patient Extension instructions for the extension process. An email will be sent to you when it is time to renew your card. Sign into the system using your username and password.

Starting MCPP Patient Renewal Application

You need to select a 1-year, 2-years’, or 3-years’ timeframe for the renewal application. Note: If your medical condition is lifelong, the timeframe will be automatically set by the system for a three-years term. You will have to answer the caregiver question.

Cancelling or Adding a Caregiver in the Renewal Application

If you are not on the MCPP Patient Caregiver tab, select the MCPP Patient Caregiver tab.
