Medical records will only be released when the requester provides proper identification. Photo identification is required. If you are requesting records for someone other than yourself, the patient must have completed the authorization form giving you authority to obtain copies.

If you or someone in your household has been tested at HRRMC for COVID-19, do not come to the hospital to obtain your results. You will be contacted with your results. The department in which you were tested will contact you with the results (whether that is the Emergency Department or your provider’s office).
