What you should bring to your new patient appointment: 1 Plan to arrive 15 minutes prior to your scheduled appointment time. 2 A valid/current copy of your insurance ID card is required at the time of your office visit. Please do not give us your cell phone with the picture of your card, we need a paper copy of the front and back of the card. If we are unable to verify your insurance information before you see the doctor, payment in full will be expected at the time of service. 3 If your insurance requires an authorization for the visit, you must obtain this prior to arriving for your appointment. 4 Completed patient information and medical history forms. 5 A photo ID 6 Forms of payment our offices accepts: Cash, Checks, Visa, Mastercard, Discover, American Express, and Care Credit.
Other forms are informational. If your doctor has given you a form with specific information or instructions different from the information on these online forms, rely on the information given to you by your doctor, not on the online forms.
Our Electronic Medical Record Company has developed a program that allows patients to access their health information through a secure internet portal, similar to logging into a bank’s website to view your checking account. This Portal is managed by an outside vendor with continuous updates to ensure that it is secure and private.
Appointments: You will be able to schedule, re-schedule, or cancel your own appointments on-line.
You can ask our front staff for a Patient Account Enablement form or print this off of our web site by clicking here. This must be signed and returned to the office prior to activating your account. This form reviews the features of the portal and the terms of the agreement.
At Asheville Medicine and Pediatrics our goal is to provide you with the most personal and up-to-date medical care available. We have always believed in shared decision making and allowing patients to have an active role in their health care.
Patient Portal communication has a number of advantages over other types of communication. It is fast, convenient, and efficient. It also works well for many of the questions, requests, or messages you may have for our practice, such as prescription refill requests, appointment scheduling and non‐urgent medical questions.
The most important thing you should know is we cannot guarantee the confidentiality of exchanges. While the security of e‐mail is comparable to other types of communication (such as phone calls), there are some special issues with e‐mail.
When something new is added to your Patient Portal, you will receive an email that simply tells you that you have updates waiting for you in the portal. The reminder emails will always include a link to the Patient Portal for quick access.
Every effort will be made to respond to our Patient Portal question or message by the next business day. You should not expect to receive a response on weekends or holidays or any other time the office may be closed.
