Getting An Appointment

We offer a wide range of outpatient primary and specialty medical services. To make an appointment, call 920-288-8000. Our staff will help connect you to appropriate clinical team and guide you through the appointment process. We will help you work with your current medical providers to obtain all pertinent records and coordinate your care.

Once scheduled, you’ll be able to see your appointment date and time in the calendar at the bottom of the main page. You can click on that calendar appointment to start the visit at the scheduled time, or you can click on the “Get Started” link from an email that you’ll receive.
