With the Patient Portal, you enjoy secure access to your information and can interact with your account, request appointments, request prescription refills, pay statements online and access forms from home. Your information is safe, confidential and available 24/7.

Passwords should include numbers and special characters, and be between six and twenty characters. If you are given an option to save your password in your browser, do not do so. Make sure to logout after accessing the Patient Portal.

A routine eye exam is performed to detect any health issues that might be threatening to your eyes or vision. The exam includes a refraction test to detect the need for corrective lenses.

As part of a medical practice, Horizon Eye Care doctors are sometimes called upon for eye-related emergencies throughout the day. These emergencies and other special circumstances may also add to your wait time. We regret the inconvenience, but appreciate your patience, in these situations.
Please check in at the front desk when you arrive. If you filled out the necessary paperwork before arriving, please give that to the receptionist. If not, be prepared to fill out the required forms in the waiting room.
