The first time you log into MyChart you will be required to create a unique User ID and a six-character password. It is extremely important that you keep your login name and password completely confidential. MyChart users must be 18 years of age or older to request a personal account, with the exception of emancipated minors who provide written documentation to prove their emancipated status.
