What is the Holy Name Medical Center?

The Health Information Management Services (Medical Records) Department is responsible for managing requests and releasing copies of medical records. Due to the confidential nature of a patient's medical record, Holy Name Medical Center requires ALL requests for the release of medical records be accompanied by a completed, HIPAA-compliant, Authorization for Release of Medical Information Form, signed by the patient or legal representative.

How long does it take to get a copy of a medical record?

to 4:30 p.m. Note: A photo I.D. is required at the time of your request. HIPAA regulations allow a healthcare entity up to 30 days to process copy requests for medical records.

Patient Advocate

Medical Centerization and illness can be very stressful and disruptive for the patient/family/other (s). There are occasions when you may need someone other than the Medical Center staff to speak for you.

Compliments or complaints can be made by meeting with the Patient Advocate or by contacting via mail, email or telephone. You do not need to identify yourself, but if you do it will help to identify and correct problems and will validate the complaint/compliment.
