Federal law prohibits UHS from including the television and telephone charges in your daily room rate. This service is provided by Pathway Solutions, LLC 3840 East Robinson Rd, Suite 140, Amherst, NY 14228. Pathway’s telephone number is 888.891.6523. Costs for long-distance calls can’t be charged to your room.

For Patients. If you’re admitted to the hospital by your physician, you’ll receive a phone call to let you know when the hospital can admit you. If you’re admitted directly from your physician’s office, please report to Admissions unless your physician directs you otherwise.
Our nursing staff and a care manager or social worker will assist you with discharge plans. These plans start with information collected on admission and are evaluated for you by nursing staff, your physician and case manager or social worker throughout your hospital stay. If you have concerns regarding your discharge plans, contact your physician, nurse, case manager or social worker.
To obtain copies of your medical records, please contact the clinic or hospital where you received care and ask to speak with the Medical Records department. The UHS staff will be happy to assist you. A list of all UHS clinics and hospitals and their phone numbers is available online. Or, fill out a Medical Record Authorization form.
State and federal laws determine who may have access to a patient’s information. If you are a parent or legal guardian of a minor child, in most cases you will be able to access the child’s health care information if you provide proper identification and sign a consent form. Legal representatives can also gain access to patients' records. The UHS Medical Records staff can answer any questions you might have regarding access to family members' information.
If you have questions about your health insurance or your hospital bill, a Patient Financial Advocate will be happy to answer your questions by telephone, or if you request it, will come to your room while you’re still in the hospital.
If you’re interested in quitting smoking while you’re a patient in the hospital, UHS offers a nicotine consultation service at no charge. Talk to your nurse or physician if you need a referral. Interpreter Services: UHS will provide an interpreter for any patient or family who doesn’t speak English.

Bring your health insurance card or a copy of it. Program the telephone numbers for the Decker Student Health Services Center (DSHSC) (607-777-2221) and Harpur’s Ferry ambulance service (607-777-3333) into your phone.
If you take prescription medication on an ongoing basis, be sure to bring enough with you, or have your doctor call your prescription into a local pharmacy.
You must complete and submit both the online Medical/Health History/Tuberculosis questionnaire and the Certificate of Immunization Worksheet with immunization records before you will be eligible to register for classes.
One of the features of MyChart is Proxy Access. Obtaining access to another persons’ UHS MyChart account that is NOT their own is called Proxy Access.
UHS is excited to offer you the enhanced MyChart patient portal experience to better manage your health! Please note, your UHS medical information for approximately the past 3 years only will be in your MyChart account. If you have questions about your prior medical history, please talk with your provider.

Option 1: You can enroll in the Ascension Lourdes Patient Portal by talking to your Ascension Lourdes Provider’s office staff or the Hospital’s Registration Department staff, in person. You will receive your e-mailed invitation to join the portal from our staff. To complete your registration, an e-mail invitation with a secure, unique link will be sent to you. When you click the link, confirm your name, enter the 4 digit number you requested of our staff when they sent the email, date of birth and ZIP code. Next, choose a username and password. You will then be asked to answer security questions in case you ever forget your password.
