How do I enroll in FollowMyHealth?

If you would like to begin enrollment in FollowMyHealth sooner, or have no pending services scheduled, please feel free to visit any Clinic facility and see your receptionist at your convenience to enroll. It just takes a few minutes to fill out the form, and we try to process these as quickly as possible.

For patients who are: 1. Discharged from the Emergency Department or hospital inpatient areas while your COVID-19 test results are pending or 2. Tested at our drive-up testing facility
