To engage our patients even more in their care and to comply with the 21st Century Cures Act, starting April 2021 we start sharing most of the information found in your health record to the Follow My Health and MyHealth patient portals immediately after they are finalized. When it’s time for your appointment open the HHHN app (or log in from your computer) then tap or click on “Start Video Call”. Wait until your provider starts – don’t navigate off this screen. You’ll be able to see and talk with your provider (and nurse). When your visit is over, close the app or browser page.
