How to access patient portal?

Registering for the patient portal is simple. Just provide a preferred email address to the HFP staff and we will send you an invitation to the Harvard Family Patient Portal. We will need to know what kind of Patient Portal account you would like us to prepare for you. There are the two different account types…. Individual Account.

How do I enroll in the patient portal?

Non-HarvardKey Holders: Log in with your Username assigned to you by HUHS Health Information Services/Medical Records. Patients who do not have a portal account can create one by sending an email request to mrecords@huhs.harvard.edu and one of our HIS team members will reach out to you directly to provide assistance.

Bringing patients and providers online with the Harvard Dental Center Patient Portal

The Harvard Dental Center Patient Portal is an online module that allows patients to connect to their electronic health record through the Internet.

Patient Forms - Please allow 24 hours for medical history forms to appear in your new online account

The Harvard Dental Center Patient Portal allows you to provide information for your health record online from any internet enabled device. Once filled, the information is directly stored and can be reviewed by your Harvard Dental Center care providers.

Confirm Appointments

Patients can quickly confirm their appointments with a single click. The information is sent directly to the center and will update the schedule.

Appointment History

You can view your past and future appointments, the date and time of the appointment as well as the doctor that will be providing care for that appointment. This will give you all the necessary information about your appointment before you arrive at the center.

Registering for Access

Please email hdc_portal@hsdm.harvard.edu if you are having any issues registering for access. You must have a valid name, date of birth and email address in our electronic health record in order to register.

Online Patient Portal

Harvard Family Medicine offers a secure online portal for sharing medical information with patients. Visit our portal at harvardfamily.myupdox.com. To access your information, you will need an ID and password provided by our office, or a Fax ID and password for viewing a fax from our office.

Insurance & Payment Polices

We accept most insurance plans. Please contact our office to confirm. The patient (or legal guardian in case of a minor) is responsible for understanding the coverage of the insurance policy. Co-Pays are due at the time of your visit.
